What a full audit produces
This is a walkthrough of the report structure a measured engagement delivers, built entirely from synthetic data so the format can be reviewed before any real engagement begins.

Afia Index

Illustrative sample using synthetic data. Not a client result, benchmark, prediction or clinical classification.
What the Index is, and is not
The Afia Index is the unweighted average of seven pillar scores, each rescaled to 0–100. It is a measured result from a specific, dated survey window not a certification, and not comparable between organisations, because no benchmark dataset exists.
Every segmented figure in this report carries its response rate alongside it. Below 40% response, departmental findings are labelled indicative only. No group under five responses is ever reported separately.
The Index, and all seven pillars in one view
Physical and Metabolic Health

Mental Health and Stress

Psychological Wellbeing

Financial Wellness

All seven pillars are weighted equally. Pillar scores are always published beside the Index so nothing hides inside the average.

By site, shift, department and pay band
Subject to the suppression rule: any group below five responses is merged into a wider group rather than reported alone, so nobody can be identified by elimination.
| Segment | Responses | Index |
|---|---|---|
| Head office | 62 | 59 |
| Site A operations | 41 | 49 |
| Site B warehouse | 38 | 44 |
| Night shift (all sites) | <5 | Suppressed merged above |
From the three free-text questions
Every comment is anonymised by a person before use. If a comment cannot be made anonymous, it is not used. The lines below are illustrative composites, not real responses.
“Workload spikes around quarter-end make it hard to switch off, even with flexible hours on paper.”
“I know the benefits exist. I have never been shown how to use them.”
“My manager would listen if I raised something. I am just not sure who else would.”
What needs attention, ranked
| Risk | Pillar | Owner |
|---|---|---|
| Rest is not restorative for a large share of respondents | Recovery, Prevention & Longevity | [Named owner] |
| Financial benefits exist but are not understood | Financial Wellness | [Named owner] |
| Manager capability inconsistent across sites | Work, Culture & Leadership | [Named owner] |

What this report can and cannot conclude
- Instrument: 21-question employee survey, seven pillars, three reverse-scored items per pillar.
- Response rate: Printed on the cover and beside every segmented figure.
- Below 40% response, departmental findings are labelled indicative only.
- Responses excluded for straight-lining are counted and disclosed, never silently dropped.
- This instrument is not benchmarked against other organisations, and does not diagnose or predict any individual outcome.
